You would have to take main manifold off head and pull gaskets off and inspect I've seen sand clog it before real fine sand

have you pulled all the water injection screws out of the pipe and poked a drill bit through to ensure they arent blocked over with carbon so even if you think you are injecting water you arent? otherwise it sounds like the exhaust water jackets are blocked causing the manifold to overheat
