Were you running a small washer inside the case bushing instead of a large washer on the bendix shaft ?
Well... I picked up a tap set at harbor frieght for a quick use.... Seems like they are just a tad too small. I got one to slightly thread but the threads stripped when trying to pull it out... any other ideas to get this out... It seems like its' really in there.....
Becareful with the tap method. Its works well, right up until it doesn't, and then you have real problems.

Get a small ball end dremel type bit, you can use it with a drill if you don’t have a dremel. Cut a trench down the length of the old bushing. That will relieve the pressure of the fitment and you can pick out the old bushing. You may have to do two cuts 180 out from each other. Don’t worry if you slightly go into the case material it won’t affect the seating of the new bushing. And yes, I have actually done this.
Where are you at in So cal?
My shop is in Upland, I have plenty of those bushings in stock for my covers and cases. Stop by and Ill even remove/install it for you if you want.
What about that crack? I would be more worried about that really,
