Need Waiver Help

I need to make a simple Waiver for my Clinics. I know all the freeriders coming are cool as hell. Safe than sorry though.

Does anyone have, or know where I can get a Waiver for clinics ?
Without having lawyer type one up or paying 30 online download ?
